Data sources, provenance, and licensing

Training data, fine-tuning data, evaluation data, and any operational/inference-time data. Note licensing, consent, and any sensitive categories.

What internal accountability exists today, and what has gone wrong before.

Decision rights, model release process, sign-off chain, red-team practice, post-deployment monitoring.

Incident or near-miss history

Optional but strongly encouraged. Concealment discovered during certification is grounds for refusal.
